Request an Outreach Activity

To request an outreach activity, please complete the following Outreach Request form or call (309) 438-3655 and ask to speak with the Outreach Coordinator.

To increase the likelihood that staff will be available for your event, please provide us with a minimum of three weeks notice for all outreach requests. If your department/organization is able to offer flexibility in dates, please note that below. Availability of outreach events is subject to staff availability and staff expertise.

***If your outreach request involves crisis intervention debriefing, please call SCS at (309) 438-3655 and ask for the on-call therapist.

* You will receive a response via email or phone within 2 business days after your form is received.
